Default Special issuance for UC

Hey Doc:
I recently received a 6-year authorization for special issuance and a special issuance medical for ulcerative colitis. The letter that I received states that the medical expires Jan-09. First question, do I need to submit any additional information to my AME to continue the use of the medical or am I set until Jan?

Second, the letter also states that due to my medical history of UC operation of aircraft is prohibited at any time new symptoms or adverse changes occur or if I experience side effect from medication and that I must report any adverse changes in my medical condition to the AMCD. Does this mean that I am ground each time my UC flares up and do I need to report it?

Lastly, In the same letter I received a form "UC specifications" for my AME. Is this report to be given to him immediatly or the next time my medical is due? (I guess this is the same subject as the first question..oops)

The UC report is required to be provided to the AME when the next physical is done. If the information looks OK, the AME can issue the medical with a NOTVALID FOR ANY CLASS AFTER JANUARY 31, 2010. and so on each year until the SI is renewed by the FAA.

Yes, you need to report it when you have a flare. They want the diseas eto be well enough controoled there are few to no flares.
