Default Which documents to obtain?

I have recently (4 months ago) stopped taking an anti-depressant after being on the medication for 1 year. I would like to start the process of renewing my medical and I am trying to bring all the required information to the initial visit with the AME to speed the process. My question is: What should I specifically ask for from my treating Dr.? Also, I initially saw a psychologist for my condition and he suggested that it might help if I try an anti-depressant, I then obtained the prescription from my normal Dr. Do I need information from both of these doctors or just the one who issued the prescription? I am just trying to get an idea of what specifics I need from both doctors so I can have them in hand when I see the AME. Thanks for the help Doc!

Get the records from both doctors and a current evaluation by the psychologist stating you no longer suffer from depression and no longer require antidepressants.
