Default Medical Application Question

I am getting ready to make an appointment for my first class medical. I hold a current second class currently. During the past three years I worked at a factory. If I was not feeling 100% or just wanted the day off, I would go visit a doctor. I really hated my job. This occured about 30 times. Yes, I know its terrible. I have since changed my ways and have left the factory (by choice). I got my second class medical almost a year ago. I have not gone to the doctor since I obtained my second class. Now when I filled out the application for my second class, I made sure that I provided proof (Charts) of EVERY visit to the doctor in the previous three years. As time goes on, those visits are slowly dropping off and exceed the three years required. My question is this: When filling out the application for my first class, Do I once again have to submit all documents in which I had previously submitted to obtain my second class? Keep in mind, that I have NOT gone to the doctor since my last Medical Exam. Is there a specific thing I can write in question #19 to indicate the documents are already in the FAA's pocession? Thanks so much!!!

I know to put "Previously Reported, No Change" for the question above. Where you have to check all of the individual boxes, but this is for the previous 3 years medical history.... Just wanted to clarify that. Thanks!

For question 19, the time frame is only 3 years. You put down nothing if it is more than 3 years.

If you have a medical problem where you answered positive in one of the question 18 boxes, then put down PREVIOSLY REPORTED, NO CHANGE. You will need to tell the AME what these were since he will have to make a comment in Box 60 regarding these answers.
