Not necessarily. If there's an NDB-A, then you won't have a VOR-A. Or if there's a VOR-A at another airport for the same city, you wouldn't get one too.

Yes, you must do the course reversal. Three reasons:
1) The segment from the VOR to MONAY is the intermediate segment and there's a steady ramping down of the protected area from the VOR to the FAF. If you come in at a high speed with a tailwind, you may exceed the protected area for the large turn you have to make and hit something.
2) One of the reasons for an intermediate segment is to get the aircraft aligned and stabilized in preparing for final descent. You'd be cutting that short to do as you suggest, increasing the possibility of a poorly executed approach.
3) It's against the law.
