ASAP isn't negative, the ASAP coordinator takes the names off the report to make it anonymous and then the committee meets to discuss what happened and how it could have been prevented. There is a time frame (usually 24 hours) that you have to submit the report. An ASAP won't go in your company "file", but a violation will certainly go on your record.
