Exactly why do you "need to know" the systems of an aircraft to play a computer game?

I'd like to know when Microsoft is going to develop "Surgery Simulator 2007" so that I can second-guess every doctor in the world. Maybe we'll be able to download ER or Grey's Anatomy scenery to make it more realistic.
