Wow !!!!!! Not only have you posted the questions, but you have posted your responses and then went ahead and fully identified yourself.
There are many lurkers out there and I would imagine some from ExpressJet as well. I cannot imagine they are too happy with posting these things. And even if they still don't know who it is, it would not be very hard to figure it out. With the push of a few keys I just found out your name/address/FAA info.

Unless I am missing something, you should probably have had the name changed on the LOR. I know that shouldn't change the thought behind teh recommendation and that may seem like a minor detail, but that seems like a big No-No.
Seriously though...I wish you the best in your job search, but this along with some of the comments in you responses may not look the best to a potential interviewer.
