Most (all?) modifications of that nature would require a Supplemental Type Certificate, and yes the AFM/POH/Placards/Limitations would have to reflect the new data.
When the airplane was made it came with an Airworthiness Certificate. This Airworthiness Certificate validates that the aircraft has been inspected and conforms to the type certificate, and is in condition for safe operation.
The type certificate above is the TCDS (Type Certificate Data Sheet).
If something is modified, the airplane is no longer airworthy unless the aircraft data is amended. Methods of modification that I know of are through either an STC, FAA Field Approval, or an Airworthiness Directive.
